Are Nic Salts Better Than Cigarettes? Comparing the Health Effects Nicotine salts have become one of the most widely used forms of nicotine e-liquid, particularly among people moving away from cigarettes. Their ability to deliver nicotine efficiently raises an obvious question: are nic salts better than cigarettes? From a health perspective, the evidence points clearly in one direction. For an adult who already smokes, switching completely from cigarettes to a regulated nicotine vape is substantially less harmful than continuing to smoke. That does not make nic salts harmless, though. They still contain addictive nicotine and vaping introduces substances into the lungs that would not be present if somebody neither smoked nor vaped. The crucial difference is combustion. Cigarettes burn tobacco. Nic salt vapes heat e-liquid without burning tobacco, avoiding many of the toxic chemicals created during combustion. The NHS states that vaping poses a small fraction of the risks of smoking in the short and medium term, while making clear that it is not completely risk-free. If you currently smoke cigarettes, however, completely replacing smoking with a regulated nicotine vape can substantially reduce exposure to many of the substances responsible for smoking-related disease. Why Are Cigarettes So Harmful? Nicotine gets much of the attention when people talk about smoking, but it is not the main reason cigarettes cause such a large amount of disease. The problem is what happens when tobacco burns. Cigarette smoke is an extremely complex mixture of gases, particles and chemicals. According to the World Health Organization's overview of the health effects of tobacco, smoked tobacco products contain more than 7,000 chemicals, including at least 250 substances known to be toxic or carcinogenic. The WHO describes tobacco use as a major cause of cancer, heart disease, stroke and respiratory disease, with tobacco damaging nearly every organ in the body. Smoking and Cancer The relationship between smoking and cancer is well established. Tobacco smoke exposes the body to numerous carcinogenic substances over and over again, often for years or decades. Lung cancer is the most obvious example, but the effects are not limited to the lungs. Tobacco use is associated with numerous cancers throughout the body. The WHO reports that smokers can be up to 22 times more likely to develop lung cancer than non-smokers. Smoking and Your Lungs Cigarette smoke repeatedly exposes the airways and lung tissue to an enormous mixture of irritants and toxic chemicals. Over time, this can produce persistent inflammation and permanent damage. One of the clearest examples is chronic obstructive pulmonary disease, or COPD. The NHS identifies smoking as the main cause of COPD and estimates it is responsible for around nine in ten cases. Once significant COPD-related lung damage has occurred, it cannot simply be reversed. Smoking and Your Heart Smoking is also deeply tied to cardiovascular disease. It damages blood vessels, affects circulation and increases the likelihood of heart attacks and strokes. According to the WHO's European tobacco health data, smokers are around two to four times more likely than non-smokers to suffer coronary heart disease and stroke. What Are Nic Salts? Nic salts, or nicotine salts, are a form of nicotine used in e-liquid. They differ chemically from traditional freebase nicotine. Freebase nicotine is relatively alkaline and can feel increasingly harsh as its concentration rises. Nicotine salt formulations mix freebase nicotine with acid, commonly benzoic acid, to create a protonated form of nicotine with different chemical and sensory characteristics. One practical result is a smoother sensation at comparatively high nicotine concentrations. A randomised clinical trial comparing nicotine salt and freebase formulations found that salt formulations were rated as smoother and less harsh than freebase nicotine. This is important because it allows a relatively small vaping device to provide nicotine in a way that some cigarette smokers find more satisfactory. It also means nic salts deserve careful consideration when discussing nicotine dependence. What Are the Health Implications of Nic Salts? Nic salts should not be described as harmless. Vaping means inhaling aerosol into the lungs, and nicotine itself is addictive. What matters when comparing nic salts with cigarettes, however, is relative risk. Asking whether vaping carries any risk at all is a different question from asking whether it carries as much risk as smoking. The Office for Health Improvement and Disparities' comprehensive evidence review concluded that vaping poses only a small fraction of the risks of smoking in the short and medium term. It also found significantly lower exposure to numerous harmful substances among people who vape compared with people who smoke. Nicotine Addiction and Nicotine Delivery Nicotine salts can deliver nicotine efficiently. This is part of their appeal to smokers, but it is also why they can maintain nicotine dependence. Nicotine delivery is affected by more than the nicotine number printed on the bottle. Device power, aerosol production, nicotine formulation, puff length, frequency of use and an individual's familiarity with vaping can all change the amount of nicotine absorbed. The UK government's 2022 evidence review on nicotine vaping found that experienced adult vapers can achieve nicotine exposure comparable with smoking under some circumstances. It also specifically identified nicotine salt liquids as one factor capable of increasing nicotine exposure. A 2024 randomised crossover study published in Nicotine & Tobacco Research found that a 20mg/ml nicotine salt formulation produced higher peak serum nicotine than a 20mg/ml freebase formulation under the study conditions. Important distinction: efficient nicotine delivery does not mean nic salts have the same overall health risk as cigarettes. Nicotine exposure and exposure to cigarette smoke are not interchangeable measures. Does Similar Nicotine Delivery Make Nic Salts as Harmful as Cigarettes? No. This is one of the easiest parts of the comparison to misunderstand. Imagine two products that deliver similar quantities of nicotine. One delivers that nicotine alongside smoke created by burning tobacco; the other delivers it without tobacco combustion. The nicotine exposure might be similar while the overall chemical exposure is completely different. The NHS explains that nicotine is highly addictive but that most of the serious harm from smoking comes from the other toxic chemicals present in cigarette smoke. Nicotine should still be treated seriously. It sustains dependence, can produce acute effects such as changes in heart rate and blood pressure, and is particularly inappropriate for people who would otherwise not use nicotine. But comparing cigarettes and nic salts solely by how much nicotine they deliver misses the primary reason cigarettes are so dangerous: burning tobacco produces a toxic mixture that vaping does not reproduce in the same way. What Do Nic Salts Do to Your Lungs? Vaping is not the same as breathing clean air. Aerosol enters the respiratory system and can expose the lungs to potentially irritating or harmful substances. The available evidence does, however, show a substantial difference between vaping and smoking. In its major review of vaping-related health evidence, the Office for Health Improvement and Disparities found significantly lower exposure to most respiratory toxicants from vaping than from cigarette smoking under typical conditions. That does not prove that vaping has zero long-term respiratory consequences. The evidence base is simply not old enough to provide the same decades-long disease data that exists for cigarettes. This is why the most accurate conclusion sits somewhere between two misleading extremes: It is inaccurate to say vaping is harmless to the lungs. It is also inaccurate to treat vaping and smoking as though they expose the lungs to the same level of harm. Current evidence supports a substantial reduction in exposure when smoking is completely replaced with vaping, while the exact lifetime respiratory risk of vaping remains uncertain. Nic Salts, Cigarettes and Cancer Cigarettes are a known major cause of cancer. Nic salt vaping does not have an equivalent body of long-term epidemiological evidence because widespread vaping simply has not existed for as many decades as cigarette smoking. That means nobody can currently give a reliable lifetime cancer-risk percentage for exclusive nic salt vaping. What researchers can measure is exposure to substances associated with cancer. On this measure, the difference from smoking is substantial. The UK government's systematic review of biomarkers found substantially lower exposure among vapers than smokers to several tobacco-specific nitrosamines and other toxicants associated with cancer risk. This does not translate neatly into a statement such as "vaping reduces cancer risk by exactly X%". Cancer develops over long periods and depends on more than a single chemical or biomarker. The evidence instead supports a more careful conclusion: nicotine vaping substantially reduces exposure to many carcinogenic substances found in cigarette smoke, but its precise lifetime cancer risk is not yet known. Nic Salts and Cardiovascular Health The cardiovascular comparison is more complicated than simply saying that vaping has no effect on the heart. Nicotine and vaping can produce acute cardiovascular changes, and studies have reported short-term effects on measures such as heart rate, blood vessels and endothelial function. At the same time, cigarette smoking exposes the cardiovascular system to a much broader mixture of combustion products, including carbon monoxide and numerous toxic chemicals. The OHID evidence review found that acute vaping can affect endothelial function, but also highlighted a randomised controlled trial in which smokers who switched to vaping showed improved flow-mediated dilation after four weeks. The review concluded that the exact cardiovascular risk associated with long-term vaping remains uncertain, although based on the toxicant profile of vaping products and aerosols, it is expected to be considerably lower than that of cigarette smoking. Nic Salts vs Cigarettes: Toxicant Exposure One of the most useful ways of comparing cigarettes and vaping is to measure biomarkers: substances or their metabolites detected in the body after exposure. The UK government's 2022 evidence review analysed numerous studies comparing biomarkers in people who vape and people who smoke. Most volatile organic compound biomarkers assessed were significantly lower among vapers. Some of the larger differences included: Chemical / Biomarker Why It Matters Exposure Among Vapers vs Smokers 3-HPMA Metabolite associated with exposure to acrolein Approximately 71% lower CNEMA Metabolite associated with acrylonitrile exposure Approximately 94% lower MHBMA Metabolite associated with 1,3-butadiene exposure Approximately 83% lower These figures should not be interpreted as a direct percentage reduction in disease risk. A 94% reduction in one biomarker does not mean a 94% reduction in overall health risk. They do, however, provide physical evidence that completely replacing smoking with vaping can dramatically alter a person's exposure to a number of important cigarette-related toxicants. The review also found some areas where results were mixed or inconclusive, including certain measurements involving formaldehyde, toluene and some metals. Vaping therefore cannot reasonably be described as free from harmful chemical exposure. What About Carbon Monoxide? Carbon monoxide is another major difference between smoking and vaping. It is created during combustion. Cigarette smoking therefore exposes users to carbon monoxide, which interferes with the blood's ability to carry oxygen. The NHS notes that vapes do not produce the tar or carbon monoxide generated by cigarette smoking. Similarly, the OHID evidence review found significantly lower carbon monoxide exposure among vapers than smokers. This is another example of why comparing cigarettes and nic salts based purely on nicotine does not tell the full story. The nicotine might remain, but an important product of tobacco combustion disappears when cigarettes are removed. Nic Salt Addiction vs Cigarette Addiction Both products can maintain nicotine dependence. Cigarettes deliver nicotine quickly and repeatedly, which is one of the reasons smoking can become so difficult to stop. Nic salts were developed partly because their smoother formulation can allow efficient nicotine delivery without the harshness associated with high concentrations of freebase nicotine. A controlled pharmacokinetic study comparing nicotine salt and freebase formulations demonstrated that the chemical formulation itself can influence nicotine absorption. That can be useful for an adult smoker who needs sufficient nicotine to control cigarette cravings. It can also make sustained dependence possible. It is therefore sensible to separate two questions: Can nic salts be addictive? Yes. Does that mean they carry the same overall health risk as smoking? No. Addiction and toxicity overlap, but they are not the same thing. Second-Hand Cigarette Smoke vs Second-Hand Vape Aerosol Cigarette smoking does not only expose the smoker. Second-hand tobacco smoke is itself an established health hazard. The World Health Organization estimates that second-hand tobacco smoke contributes to more than 1.6 million deaths among non-smokers globally each year. Second-hand vape aerosol is chemically different and generally exposes bystanders to substantially lower levels of many toxic substances than cigarette smoke. The UK evidence review found no significant increase in toxicant biomarkers following short-term second-hand vaping exposure in the studies it assessed, although the evidence base was limited. Lower exposure does not mean people should deliberately expose others to vape aerosol unnecessarily, particularly children, babies or people with respiratory conditions. It does mean that second-hand cigarette smoke and second-hand vape aerosol shouldn't be treated as equivalent exposures. Are Nic Salts Better Than Cigarettes? If by "better" we mean less harmful to the health of an existing adult smoker, current evidence strongly supports the answer being yes. The primary reason is not that nicotine salts are harmless. It is that they provide nicotine without requiring tobacco to be burned. The NHS states that people who switch completely from smoking to vaping reduce their exposure to toxins associated with cancer, lung disease, heart disease and stroke. The Royal College of Physicians' 2024 evidence review similarly recognises e-cigarettes as an important tool for reducing the health burden caused by tobacco, while stressing that vaping should not be encouraged among young people and people who do not smoke. So, for somebody who currently smokes cigarettes, the comparison looks very different from the comparison for somebody who uses no nicotine at all. For a smoker: completely switching from cigarettes to regulated nicotine vaping is expected to substantially reduce harm. For a non-smoker: starting to vape introduces nicotine dependence and avoidable exposure where none previously existed. Are Nic Salts Healthy? No. "Less harmful than cigarettes" and "healthy" are not the same statement. A person who neither smokes nor vapes generally has less exposure to nicotine and vape aerosol than someone who uses nic salts. There is no health advantage to taking up vaping if you would otherwise use neither product. The relative advantage of nic salts appears when they replace something substantially more harmful: combustible cigarettes. This distinction is central to the UK government's assessment of vaping risks. Vaping is not risk-free, particularly for people who have never smoked, but its short and medium-term risks represent a small fraction of those associated with smoking. Does Switching to Nic Salts Reduce the Harm From Smoking? The biggest potential health benefit comes from completely replacing cigarettes. Continuing to smoke while also vaping means cigarette-smoke exposure is still occurring. A person may reduce the number of cigarettes they smoke, but they have not removed combustion from the equation. Complete switching matters because the toxicants associated with burning tobacco disappear or fall dramatically once cigarettes are removed. The body's recovery from smoking can begin remarkably quickly. According to the NHS guide to the health benefits of quitting smoking: After around one year, the risk of heart attack is around half that of somebody who continues to smoke. After around ten years, the risk of death from lung cancer falls to around half that of a continuing smoker. After around fifteen years, heart attack risk falls to approximately that of somebody who has never smoked. Vaping is not required to achieve those long-term benefits; stopping smoking is the important part. For some smokers, however, nicotine vaping can provide a practical route away from cigarettes. Nic Salts vs Cigarettes: The Bigger Picture Question Cigarettes Nic Salts Do they contain nicotine? Yes Yes Can they cause dependence? Yes Yes Do they burn tobacco? Yes No Do they produce tobacco smoke? Yes No Do they produce tobacco tar? Yes No Are they a major source of carbon monoxide? Yes No Can nicotine delivery be substantial? No No Recommended for non-smokers? No No What We Still Don't Know About Nic Salts Cigarette smoking has been studied across generations. Researchers have decades of population data connecting smoking with cancer, COPD, cardiovascular disease and premature death. Vaping does not yet have that length of follow-up. Important unanswered questions remain around the effects of very long-term exclusive vaping, including how risks might vary according to: Device type and power output Nicotine concentration Nicotine formulation E-liquid ingredients and flavourings Frequency and intensity of use Length of exposure Changes in vaping technology over time Individual health conditions and susceptibility This uncertainty matters. It is why credible health organisations do not describe vaping as harmless. But uncertainty should not be turned into an assumption that the risks are therefore equal to smoking. The most comprehensive UK evidence review to date found substantially lower exposure to numerous toxicants among vapers and concluded that short and medium-term vaping risks represent a small fraction of those from smoking. The Bottom Line: Are Nic Salts Better Than Cigarettes? For an adult who currently smokes cigarettes, completely switching to regulated nic salt vaping is expected to be substantially less harmful than continuing to smoke. Cigarettes deliver nicotine through the combustion of tobacco. That combustion creates carbon monoxide, tar and thousands of chemicals, including numerous substances capable of causing cancer and damaging the lungs and cardiovascular system. Nic salts remove the burning tobacco, but not the nicotine. As a result, they can still maintain nicotine dependence and vaping is not without potential health effects. Some toxicants remain present, the aerosol is not simply harmless water vapour, and the exact health consequences of vaping continuously over several decades cannot yet be measured with certainty. None of that changes the central comparison. Current evidence from the NHS, Office for Health Improvement and Disparities and Royal College of Physicians supports vaping as substantially less harmful than smoking for adults who switch completely. The healthiest option remains using neither cigarettes nor nicotine vapes. But when the choice being compared is continued cigarette smoking versus replacing those cigarettes with a regulated nicotine vape, they are not two versions of the same risk.
